Assist-Life Stage, Transition

A new phase of life might be difficult to adjust to. For this reason, Ivy Community Care is available to help people as they go through changes in their lives. We provide a wide range of Assist Life Stage Transitions services, such as helping clients move into supported housing or into a new school, as well as helping them get out and about in the community.

Our disability services team can help you to identify your needs and make a plan to ensure the best possible outcome. This includes developing a plan for moving into a new living setting, finding and securing suitable job or training opportunities and accessing community support services.

Community Nursing Care

A licensed nurse can offer our community nursing care services on a regular or as-needed basis at your home or in our community.

For qualified NDIS participants whose NDIS Plan involves community nursing capacity building, NDIS Community Nursing Care is offered.

This program offers support with wound care, chronic condition management, general nursing care to keep you healthy, and hospital-quality treatment outside of a hospital.

Our nurses’ mission is to provide you with outstanding clinical care while enabling you to live freely in your own home.

Daily Tasks/Shared Living

Ivy Community Care provides Daily Tasks/Shared Living support services to help individuals with disabilities live independently and comfortably in a shared living environment.

Ivy Community Care understands that moving into shared living can be a difficult adjustment, we provide individualized attention. We collaborate carefully with each participant to fully grasp their individual needs and preferences before offering help that is customized to fulfill their needs.

In addition to helping with personal hygiene, grooming, and dressing, our team can support you in keeping your living area tidy and orderly.

Innov Community Participation

Ivy Community Care Innovative Community Participation Program enables NDIS members to demonstrate their skills, build meaningful relationships and engage in various community activities, which will enable them to become more independent and confident in themselves.

We can assist you in developing new talents and strengthening your relationships to your community by encouraging and finding you the things you enjoy.

Development-Life Skills

As part of this support program, we assist NDIS participants and/or their carers in developing life skills and everyday living abilities. Support staff at Ivy Community Care help clients build the skills necessary to live as independently as possible.

We believe that if you have the necessary life skills, you can shape your life to be exactly what you want it to be.

We encourage our participants to take on the world and use their life skills to live as independently as possible since we are passionate supporters of self-sufficiency. Our strong work ethic and generosity serve as the foundation for our approach to improve a participant’s skill and potential.

Household Tasks

To live a happy and fulfilled life, one must keep their home tidy and pleasant. We at Ivy Community Care recognize the importance of domestic duties and responsibilities in enhancing your general health.

Our devoted group of caregivers is committed to provide thorough assistance with a variety of home duties. We make sure that your living areas are neat, cozy, and supportive of your everyday routines, from cleaning and organizing to meal preparation and laundry.

Participate Community

We work alongside you to build a community participation plan with activities that you truly enjoy. Whether it’s picking up a new skill or going shopping with someone, our staff will make sure you keep doing things that bring you joy.

Ivy Community Care works with a number of top SDA (Specialist Disability Accommodation) housing providers to make sure individuals have a place where they can truly call home. SDA helps people with complex disabilities live more independently, and more socially inclusive by offering them specialized housing options.

Group/Centre Activities
