At IVY Community Care, we maximise our disability support to meet each client’s specific needs as we recognize that disability can mean various things to different people. As a disability services provider, we understand that some of the most significant elements of the NDIS include safety, choice, respect, and high-quality care. We keep this in mind as we provide care at the convenience of your home or at any other place of your choosing. Ivy Community Care consists of a group of skilled, competent and knowledgeable nurses, support workers, and other healthcare professionals who have joined forces to offer excellent support to people with disabilities. We assist individuals with disabilities to live more freely in the environment of their choice and to take an active part in society.
Participants at Ivy Community Care have greater freedom and choice about the kind, frequency, and intensity of their support. Our approach centers on the unique preferences and support requirements of each participant. Our philosophy of consumer-directed care is in accord with this.
When clients join Ivy Community Care, we want to hear about their experiences and support them as they begin a new chapter.We offer a customized support plan detailing their aims, obstacles, and goals. We help participants grasp their plans and maximize their bundle without compromising the caliber of our services.
Everything we do is focused on the needs of the participant. Our care coordinators collaborate closely with clients to determine what services they need in order to get the best potential outcome. Because of this, we can maintain our long-term viability and continue to offer top-notch service both today and in the future.
